What we do for you

Replacing a child's lost passport combines two sets of paperwork: the lost-passport forms, with a police report and Annexure F, and the parents' consent, with both parents signing Annexure D.

We prepare every form for you to sign, check each signature against your passports and tell you what to do if the old passport turns up.

  • Annexure F and Annexure D prepared
  • Signatures checked against both parents' passports
  • Help getting the right police report
  • Photographs printed to the checklist size

Who it’s for: Children under 18 whose Indian passport is lost, stolen or damaged

Which applies to you?

It depends on your circumstances

When you start, we go through these with you.

Report it to the police

A lost or stolen passport must be reported to the police straight away. A damaged passport does not need a police report.

Lost or stolen
Both parents sign

Annexure D is signed by both parents, and each signature must match that parent's passport.

Signatures must match
When one parent has not given consent

Annexure C covers a single parent. For a re-issue, custody papers from a UK court are not accepted; an Indian court order is needed, or both parents sign Annexure D.

Annexure C
No name change

The child's name cannot be changed when a passport is replaced because it was lost.

Same name
It takes longer

Checking the old passport record can take four to six weeks, and police verification can take three to six months where it is needed. Tatkaal is not available.
